Our Services

NJ4S provides evidence-based prevention services, brief clinical interventions, and referrals to long-term services as needed.

Services are available on-site at schools, libraries, and other community organizations, as well as virtually and hybrid.

NJ4S Passaic County staff can deliver programming in English and/or Spanish.

Three Tiers of Specialized Services

Tier 1

Universal Supports

  • Workshops, webinars, and assemblies for students, families, school staff, and community members

How to Access Our Free Services

NJ4S offers support at three levels to ensure that everyone’s needs are met, whether they are seeking general wellness education or more targeted interventions.

Tier 1 Services

Universally Available

Students, their families, school staff, and community organizations within Passaic County can request these services by filling out this Registration Form or by directly contacting our Passaic County Hub by message or phone (973-413-1555).

Tier 2 & 3 Services

Students (Grades 6-12)

To be considered for our targeted group and individual clinical services, an application must be submitted by the school’s designated NJ4S liaison.

Once the application is submitted, our staff will meet with the liaison and determine whether NJ4S services or a referral to a community resource can be provided.

How School Staff Can Request Tier 2 & 3 Services

  1. School staff must complete the NJ4S User Contacts Survey form to receive an email with Salesforce account information.

  2. After completing the survey, Applications must be submitted by school staff through Salesforce, the NJ4S referral system.

  3. Using the account information provided, school staff can sign into Salesforce and click Submit New Application.

  4. Staff can follow the prompts to fill out the School Information, Reason for Application, and Participants sections.

  5. In the Next Steps & Finish section, staff can…

    • Go back to previous sections to make edits.

    • Save the application and exit, to submit at a later time.

    • Submit the application to the NJ4S Hub.
